Drug-induced movement disorders include akathisia, extrapyramidal symptoms (EPS), and tardive dyskinesia, each with a prevalence of 30%. Acute dystonia is rarer, occurring in about 5% of cases.
Extrapyramidal symptoms (EPS), an outdated term, refer to secondary parkinsonism characterized by bradykinesia, tremors, rigidity, and reduced spontaneous movement.
Antipsychotic medications frequently cause tardive dyskinesia, a hyperkinetic movement disorder associated with dopamine receptor blockade.

Hello! I’m Dr. Greg Pontone and I’m the chief of Aging, Behavioral and Cognitive Neurology at the University of Florida. I also co-direct the Fixel Neuropsychiatry Program here and have an adjunct appointment at Johns Hopkins University. For most of my career, I’ve treated and done research on neuropsychiatric symptoms and neurodegenerative disorders. So today, I’m going to talk to you about drug-induced movement disorders, sedation and cognitive impairment caused by those same medications.

So when we think about drug-induced movement disorders caused by medications, there are four main types. Akathisia, which is often described as a sort of inner sensation of restlessness. People will say that they can’t sit still. They have to move to sort of dissipate this distressing inner feeling.

02. [FR] Akathisia: How to Diagnose and Manage?

Learning Objectives:
After completing this activity, the learner will be able to:

Identify and differentiate between the four main types of drug-induced movement disorders (akathisia, extrapyramidal symptoms, tardive dyskinesia, and acute dystonia) and select appropriate evidence-based treatments for each.
Assess patients for medication-induced cognitive impairment and implement strategies to minimize cognitive side effects while maintaining therapeutic efficacy.
Apply a systematic approach to evaluate and manage extrapyramidal symptoms, including distinguishing drug-induced parkinsonism from idiopathic Parkinson’s disease using clinical features and biomarkers when appropriate.
